Licensed Therapist – Memory Care & Cognitive Support

LMFT, LCSW, LCPC, or LMHC | Flexible 1099 Role | Established On-Site Caseload

\n

Silver Rising Therapy is seeking a compassionate, patient, and creative LCSW, LCPC, LMFT or LMHC to provide on-site emotional support and cognitive stimulation to older adults living in a dedicated memory care community in Burr Ridge.

\n

\n

About the Role

\n

This is a unique clinical role working primarily with individuals experiencing dementia, advanced cognitive decline, and age- or end-of-life-related needs .

\n

This work often looks different from traditional outpatient therapy. The focus is on meeting residents where they are cognitively and emotionally and adapting your approach based on what is most meaningful and beneficial to that individual.

\n

Depending on the resident, sessions may include:

\n

\n
• Music, familiar photographs, or reminiscence
\n
• Cognitive stimulation and simple activities or games
\n
• Sensory-based engagement
\n
• Validation, redirection, and supportive conversation
\n
• Emotional support surrounding grief, loss, aging, and life transitions
\n
• Comfort and companionship for residents with advanced dementia or limited verbal communication
\n

\n

Some residents may be highly conversational, while others may be minimally verbal or nonverbal. Sessions are tailored to each resident and may range from shorter visits to more traditional therapy sessions.

\n

\n

Success may look different with this population. Sometimes it's therapeutic progress; other times it's reducing anxiety, encouraging engagement, providing comfort, or simply creating a meaningful moment in someone's day.

\n

• Established caseload: SRT provides referrals and clients, with a full-time caseload available at this community.
\n
• Flexible scheduling: You determine your own schedule based on your availability and resident needs.
\n
• One convenient location: Your clients are located within the same community, allowing you to move from resident to resident without traveling between appointments.
\n
• Administrative support: We handle client eligibility, billing, and other administrative responsibilities so you can focus your time on providing excellent care.
\n
• Clinical autonomy: You have the flexibility to determine how to structure your day and approach each resident based on their individual clinical needs.
\n
• Training and resources: We provide training on our clinical processes, Medicare documentation, CPT coding, and Ensora EHR.
\n
• Bi-weekly compensation: Clinicians are paid based on completed sessions and documentation rather than waiting for payer reimbursement.
\n

• Exceptionally patient, compassionate, and comfortable meeting residents where they are

• Creative in finding ways to connect when traditional conversation isn't possible
\n
• Comfortable working with significant cognitive impairment and nonverbal residents
\n
• Flexible when a resident's abilities, mood, or availability change from day to day
\n
• Able to find meaning in small moments of connection and engagement
\n
• Comfortable working independently while also collaborating with community staff, families, and the SRT team
\n
• Passionate about improving quality of life for older adults
\n

\n

\n

Previous experience in memory care, dementia care, hospice, palliative care, skilled nursing, assisted living, geriatrics, or end-of-life care is highly valued, but not required.

\n

Role Responsibilities

\n

\n
• Provide individualized emotional support, cognitive stimulation, and therapeutic services to memory care residents
\n
• Adapt sessions to residents' cognitive abilities, communication styles, moods, and needs
\n
• Conduct initial evaluations and develop individualized treatment plans
\n
• Find creative ways to engage residents who may not be able to participate in traditional talk therapy
\n
• Maintain accurate and timely clinical documentation
\n
• Collaborate with community staff and interdisciplinary care teams
\n
• Communicate with families and POAs when clinically appropriate
\n
• Support residents navigating dementia, grief, loss, anxiety, depression, aging, and end-of-life transitions
\n
• Independently manage your schedule and caseload
\n

\n

Qualifications

\n

\n
• Master's degree in Social Work, Counseling, Psychology, or related field
\n
• Active independent state licensure as an LCSW, LCPC, LMFT, LMHC, or equivalent
\n
• Clinical experience preferred
\n
• Senior care, memory care, hospice, or geriatric experience preferred
\n
• Strong interpersonal and communication skills
\n
• Ability to work independently and manage your own schedule
\n
• Comfort adapting traditional clinical skills to residents with varying cognitive abilities
\n
• Proof of independent licensure and malpractice insurance
\n
• Ability to pass a background check
\n
